STANDARD OFFICE HOURS
Monday & Friday: 8 a.m. – 1 p.m.
Tuesday & Thursday: 8 a.m. – 6 p.m.
Wednesday & Saturday: by appointment
Mallette Dental is available with flexible hours to meet our patients’ needs. Evening and weekend hours are available by appointment, and Dr. Mallette can be reached after hours for access to immediate emergency care.
APPOINTMENTS
It’s our goal to have you in a treatment room inside ten minutes of your scheduled appointment! When making our schedule, we try to allow the appropriate amount of time for each appointment. Thus, we ask that you arrive at our office 5 to 10 minutes before your scheduled appointment, so that every patient has an equal chance of being seen on time.
We value your time and, unless requested, we try to keep all appointments less than one hour so that we aren’t taking any unnecessary time out of your busy days.
If you need to change your appointment, please notify us as soon as possible. We ask for at least 24-hour notice if you need to cancel your appointment. A $20.00 cancellation fee will be charged for no-shows or repeated cancellations. This will partially cover sterilization and room set-up expenses for your specific treatment. We understand certain circumstances may arise, but please be courteous to other patients needing urgent care.
YOUR FIRST VISIT
Your initial visit with Dr. Mallette begins with a review of your general health history. We ask that you provide the staff with any information regarding your medical history. This information is critical to providing you with safe care. We also discuss your needs and expectations.
Next, digital radiographs (x-rays) will be taken, as well as a thorough initial exam, oral cancer screening and complete analysis of your dental condition. Impressions of your teeth and intraoral images may also be needed to aid in diagnosis.
A complete treatment plan will be done by Dr. Mallette and presented at your follow-up visit along with his analysis of the options on treatments available to you to improve and maintain your optimal dental condition.
PAYMENTS AND INSURANCE
Mallette Dental is an independent practice that accepts ALL INSURANCES and provides FEE FOR SERVICE (FFS) dentistry.
All payments are due the day the service is performed. We accept payments in the forms of cash, check, MasterCard, Visa or Discover. We also can help you to apply for financing. A 5% professional courtesy will be extended to Senior Citizens for cash payment in full at the time of the appointment.
For patients with an overdue balance, a finance charge may be incurred at a monthly rate of 1.5%.
For patients with dental insurance, your co-payment is due on the day of treatment. The office manager will help you determine the insurance coverage available to you by reviewing your contract or by previous payment history in our computer systems. We require one signed insurance form at the first visit of the year, after which we generate computer-printed standardized forms.
Remember, professional care is provided to you — not the insurance company. Thus, the insurance company is responsible to you, and you are responsible to us. We will help you in any way we can by filing claims on your behalf. We are happy to provide this service to you.
Dr. Jeff cares about your dental health, and many insurance companies care more about saving themselves money than your optimal treatment. Therefore, when the insurance company won’t approve a payment for what Dr. Mallette recommends, we will help you by filing claims, photos and explanations for our treatment plan. But ultimately, you need to decide what is best for your health and to whom you should listen. As always, we are willing to work out financial options for any outstanding balances.
SAFETY AND CONFIDENTIALITY
Our office follows OSHA guidelines to maintain a safe, sterile environment for our patients. We also follow government HIPPA guidelines to guarantee patient confidentiality.
